Module Highlights

 

Module 1: Achieving Goals with Cardiac and Renal Protection in Mind

 

  • Setting blood glucose, blood pressure and cholesterol management goals through timely screenings and proper treatment.  
  • Learn about lifestyle intervention strategies and diabetes self-management education and support (DSMES) services.

 

Module 2: Diabetes with Cardiovascular and Kidney Complications

  • Explore current guidelines for managing type 2 diabetes and CKD.  
  • Identify options to reduce cardiovascular risk and CKD risk in people living with type 2 diabetes. 
  • Understand the evidence supporting different treatments.

 

Module 3: Secondary Prevention

  • Discover the relationship between type 2 diabetes and risk factors for patients with a history of atherosclerotic cardiovascular disease (ASCVD) or heart failure. ​ 
  • Examine guideline-directed therapies. 
  • Understand management strategies and identify possible socioeconomic and cultural barriers that may deny prevention therapies for patients with a history of ASCVD or heart failure.

 

Module 4: Lifestyle Management and Health 

 

  •  Learn about lifestyle management and motivational interviewing to help patients make positive behavioral changes, and the evidence supporting this method. 
  • Understand the role of community health workers as key team members in patient-centered care.
