Module Description
This module is designed to empower health care professionals with the knowledge and skills necessary to facilitate effective diabetes self-management in their patients through optimizing nutrition and lifestyle. 
Learning Objectives
At the end of this module, participants will be able to:
  1. Provide dietary guidance, including carbohydrate counting techniques and meal planning strategies. 
  2. Recall dietary recommendations for patients facing challenges with dietary changes. 
Module Faculty

Alison Evert, MS, RDN, CDCES, FADCES 

Alison B. Evert, MS, RDN, CDCES, FADCES, is the manager of nutrition and diabetes programs for the University of Washington (UW) Medicine primary care clinics in Seattle, WA. She leads a team of five registered dietitian nutritionists and certified diabetes care and education specialists embedded in 14 primary care clinics. Together, her team provides nutrition services and American Diabetes Association (ADA)–accredited diabetes education at all 14 clinics. 

Ms. Evert’s recent work has included a focus on panel management to reduce care gaps, the incorporation of telehealth services for nutrition and diabetes education, the development of diabetes medication adjustment protocols, and the rollout of remote patient monitoring of continuous glucose monitoring across the UW Medicine primary care network. Under her leadership, diabetes education visits have doubled, and nutrition visits have increased by 30% in the past 5 years. 

In addition to her work at UW Medicine primary care clinics, Ms. Evert served as co-chair of the ADA’s 2019 nutrition therapy consensus report. She is the author of numerous diabetes nutrition education resources for health care professionals and people with diabetes.
