Supporting Healthy Eating Habits through Continuous Glucose Monitors (CGMs)

     

Supporting Healthy Eating Habits through Continuous Glucose Monitors (CGM) is an asynchronous online education program to equip primary care providers with the knowledge and skills to leverage diabetes technologies, specifically continuous glucose monitoring, to guide and enhance their nutrition recommendations. The aim is to support healthy eating habits among individuals managing diabetes through personalized, data-driven decision-making. This program includes interactive modules with patient videos and allows learners to practice treating people living with diabetes.

 

Learning Objectives: 

  1. Identify optimal food choices that support overall heath goals/targets and how to communicate this information with patients.
  2. Guide HCP recommendations about everyday decision-making using diabetes technologies to guide nutrition recommendations.
  3. Integrate behavioral health best practices into clinical scenarios using collaborative care models, team-based approaches, and effective communication strategies.
  4. Identify evidence-based eating patterns that can be used in the management of both prediabetes and type 2 diabetes that also provides cardiovascular benefits.

Target Audience: This program is intended for PCPs including primary care physicians, physician assistants, nurse practitioners, nurses, dietitians, pharmacists, and other members of the primary care team who manage patients with diabetes.   

Credits: This program includes 5 CE modules. The CE credit value of the program will be updated as modules are released. Participants have until December 31, 2025 to complete the modules at their own pace.  


Course Expires: December 31, 2025. 

 

Module Highlights

MODULE 1

Maximizing CGM to Facilitate
Lifestyle Changes

Participants will learn the significant benefits of continuous glucose monitors (CGMs) and their pivotal role in enhancing time in range for people with diabetes. By understanding the intricacies of CGMs, participants will gain valuable insights into real-time monitoring, enabling proactive management of blood glucose levels. 

MODULE 2

Putting It All Together: Nutrition Conversations Focused on the AGP Report

Participants will learn the essential steps of evaluating continuous glucose monitor (CGM) data and Ambulatory Glucose Profile (AGP) report. Participants will learn to identify key patterns, trends, and glucose excursions within AGP report, enabling them to make informed diabetes therapy decisions. 

MODULE 3

 Analyzing The Data: Using CGM and AGP Data to Optimize Care

This module discusses continuous glucose monitoring (CGM)–based glycemic metrics and consensus metric targets, how to interpret standard CGM data, and how to assess time in range and glycemic patterns to rapidly evaluate glycemic management.

MODULE 4

Cultural Considerations for
Nutrition and CGM

Participants will focus on identifying the most suitable eating patterns tailored specifically for individuals managing diabetes. Explore groups that form the foundation of these eating patterns, aiding in better recall and comprehension. Additionally, learners will examine the intricate connection between eating patterns and blood glucose levels, providing the knowledge and tools they need to better assist people with diabetes through informed dietary choices. 

MODULE 5

Diverse Eating Patterns and CGM:
A Case-Based Patient Journey

Participants will learn to recognize the components of the Campinha-Bacote cultural competence model, a framework essential for providing culturally sensitive healthcare. Additionally, you will gain insight into identifying cultural factors that play a crucial role in affecting blood glucose levels, enabling you to offer more personalized care to diverse patient populations.
