To help you engage your team and community as an inertia buster, ADA is hosting this practical online training. During the module we will explore the importance of assessing barriers like social determinants of health, depression, diabetes distress, and health literacy. We will discuss how to consider identified barriers during treatment plan creation. We will also provide suggestions for assessment tools you can use and effective solutions for addressing patient needs and barriers, both in the clinic and through connection with critical community resources, and diabetes self-management education and support (DSMES).

Learning objectives:

• Assess patient-level barriers to diabetes self-care and care plan follow through.
• Describe the four critical times to refer to Diabetes Self- Management Education and Support (DSMES).
• Identify the steps in an effective referral for DSMES and medical nutrition therapy (MNT).
• Locate relevant community support resources to address social and emotional barriers contributing to therapeutic inertia.

	Shannon Knapp, BSN, RN, CDCES is a registered nurse and the Manager of Diabetes Care & Education at Cleveland Clinic. She has been a CDCES in inpatient, ambulatory, and home care settings. She currently coordinates an innovative team of Diabetes Care & Education Specialists covering over 20 Cleveland Clinic offices in Northeast Ohio.
	Andrew Bzowyckyj, PharmD, BCPS, CDCES is an Associate Professor at Pacific University Oregon, and he also cares for patients in the primary care setting with Legacy Medical Group in Beaverton, Oregon. His teaching, research, and practice experiences include diabetes and other metabolic conditions, health economics and policy, and leadership development.
