Description:
There is a growing body of evidence that social determinants of health (SDOH) have an impactful role on nutrition and diabetes outcomes. Community health workers (CHWs) play an important role in providing diabetes support in under-resourced communities. This training is designed for CHWs serving people with diabetes who have received a diabetes care plan from their health care provider.  The focus of this training is to provide CHWs with the basic understanding of how foods affect blood glucose, how to read a nutrition label and how to differentiate foods within cultural context for healthy meal plan choices.  Join this interactive training where CHWs will discuss case-based scenarios of how to address SDOH and patient empowerment strategies for healthy eating with diabetes.
