Module 9: Addressing Social Determinants of Health in Your Patients with Diabetes

Don’t let structural and intermediary social determinants impact the health of r your patients with diabetes.  During this module, you will learn how to identify when to refer people with or at risk for diabetes to community resources; as well as how to best access and utilize those tools.  We will review the importance of individualized treatment based on individual preferences, social context, prognosis, and comorbidities.

 

Learning objectives:

  • Define the social determinants of health.​
  • Discuss how structural and intermediary social determinants impact health equity.​
  • Identify when to refer people with or at risk for diabetes to community resources.​
  • Summarize the importance of individual treatment based on individual preferences, social context, social determinants of health prognosis, and comorbidities.

Dr. Baig is an Associate Professor at the University of Chicago in the Department of Medicine. Dr. Baig has worked on partnered research projects to improve healthcare delivery to Latino communities and is currently working with community health centers across the Midwest to implement diabetes group visits. Dr. Baig is an Associate Director of the Chicago Center for Diabetes Translation Research.
