Primary care providers, endocrinologists, diabetes educators, and other health care professionals using telehealth as a means to connect with diabetes patients.

Learning Objectives
At the end of this presentation, participants will be able to:

• Identify common issues with self-management at key stages in life

Identify strategies to improve patient self-management

 Understanding how to engage patients in self-management strategies
