Diabetes 101: Resources for Community Health Workers
Description

Target Audience
This activity is intended for physicians, physician assistants, nurse practitioners, nurses, dietitians, pharmacists, certified diabetes educators and other interprofessional health care professionals.

Learning Objective
At the end of this activity, the attendees should be able to:

• Differentiate between Type 1 and Type 2 diabetes, and pre-diabetes.
• Discuss the importance of controlling blood glucose through healthy lifestyle activities including physical activity and meal planning 
• Explain the roles that CHWs can play in diabetes management and type 2 diabetes prevention.
