This activity is intended for Primary care providers, endocrinologists, diabetes educators, and other health care professionals using telehealth as a means to connect with diabetes patients
Learning Objectives
This activity will address evidence-based, peer-reviewed clinical data and expert opinion on quality diabetes management. At the conclusion of this activity, attendees should be able to:
- Explain how social determinants of health affect diabetes care and the role that social determinants play in a patient's care and treatment
- Discuss the impact of implicit bias and systemic racism in healthcare systems
- Introduce broad solutions for addressing disparities and inequities (community health workers, plain language, etc.)
