Healthy Eating with Diabetes
Community Health Workers CE Program
The course was developed for community health workers (CHWs) to address evidence-based, peer-reviewed clinical data and expert opinion on quality diabetes management.  

Learning Objectives

  • Describe how social determinants of health (SDOH) affects nutrition and diabetes outcomes
    Discuss how foods affect blood glucose and how to read a nutrition label.
  • Differentiate foods within a cultural context for healthy meal plans.
  • Identify strategies to address SDOH and patient empowerment to improve diabetes care.

This training is designed for CHWs serving people with diabetes who have received a diabetes care plan from their health care provider.  The focus of this training is to provide CHWs with the basic understanding of how foods affect blood glucose, how to read a nutrition label and how to differentiate foods within cultural context for healthy meal plan choices. Join this interactive training where CHWs will discuss case-based scenarios of how to address SDOH and patient empowerment strategies for healthy eating with diabetes.
