Target Audience
This activity is intended for physicians, physician assistants, nurse practitioners, nurses, dietitians, pharmacists, certified diabetes educators and other interprofessional health care professionals.
Learning Objective
At the end of this activity, the attendees should be able to:
• Discuss how referral to a National Diabetes Prevention Program (National DPP) can benefit health care providers (HCPs) and patients.
• Examine types of referrals.
• Discuss best practices in referring to a National DPP and how to make the process as seamless as possible.
• Discuss how to overcome barriers to referrals.
• Discuss reimbursement.
• Define a bi-directional referral system and provide examples of the implementation of this system.
