OVERVIEW OF THE DIASERV PROGRAM

Effective diabetes care begins with empowering people to confidently manage their health. This health education program is crafted to provide health care professionals with advanced, practical approaches for delivering impactful diabetes self-management education and support (DSMES) in accordance with the Standards of Care in Diabetes (Standards of Care) from the American Diabetes Association® (ADA). Rooted in the latest evidence-based practices, our program translates clinical guidelines into actionable strategies, driving meaningful improvements in patient outcomes, enhancing quality of life, and increasing the overall efficiency of diabetes care. 

LEARNING OBJECTIVES

  • Describe the key components and critical times for DSMES delivery in alignment with the ADA’s Standards of Care.  

  • Demonstrate strategies to integrate DSMES into routine clinical practice, emphasizing the nine core content areas to improve health outcomes, improve quality of life, and reduce health care costs for people living with diabetes.  

  • Integrate key clinical competencies—including clinical management, technical skills, problem solving, culturally competent communication, behavioral goal setting, and monitoring—to deliver effective, person-centered DSMES and support continuous quality improvement in diabetes care.

MODULES

MODULE 1

Guides participants in applying the National Standards for DSMES to deliver high-quality, person-centered diabetes education. 

MODULE 2

Reviews diabetes mechanisms, distinguishes its main types, outlines major metabolic changes and symptoms, and compares type 1 and type 2 progression to aid clinical decisions. 

MODULE 3

Covers blood glucose monitoring for diabetes management, reviews devices, teaches data analysis, and supports patient goal setting for improved self-care.  

MODULE 4

Explains how healthy eating and exercise help manage diabetes, following current ADA standards. They support blood glucose management, reduce risk of complication and enhance quality of life.  

MODULE 5

Highlights medication management as an essential aspect of diabetes management and is crucial for daily care and long-term health. 

MODULE 6

Reviews acute and long-term diabetes complications, covering hypoglycemia, diabetic ketoacidosis (DKA), vs. hyperglycemic hyperosmolar state (HHS)and major vascular issues—with an emphasis on detection, prevention, and treatment. 

MODULE 7

Provides research-backed strategies for building healthy habits, focusing on motivational interviewing, problem solving, and stress management skills. 

MODULE 8

Covers psychosocial aspects of diabetes care, diabetes screening training, clinical skills, and the 7As model for patient support. 

MODULE 9

Focuses on integrating DSMES into clinical practice through facilitation and continuous quality improvement. Participants will identify responsibilities, use team strengths, and address challenges to enhance DSMES support.
