Healthy Eating with Diabetes

     

OVERVIEW

The course was developed for community health workers (CHWs) to address evidence-based, peer-reviewed clinical data and expert opinion on quality diabetes management.  

 

This training is designed for CHWs serving people with diabetes who have received a diabetes care plan from their health care provider.  The focus of this training is to provide CHWs with the basic understanding of how foods affect blood glucose, how to read a nutrition label and how to differentiate foods within cultural context for healthy meal plan choices. Join this interactive training where CHWs will discuss case-based scenarios of how to address SDOH and patient empowerment strategies for healthy eating with diabetes.

LEARNING OBJECTIVES

  1. Describe how social determinants of health (SDOH) affects nutrition and diabetes outcomes

  2. Discuss how foods affect blood glucose and how to read a nutrition label.

  3. Differentiate foods within a cultural context for healthy meal plans.

  4. Identify strategies to address SDOH and patient empowerment to improve diabetes care.
