This practical webinar is designed to help primary care teams enhance diabetes management through innovative, scalable care models. Learners will review CGM and AGP fundamentals, explore strategies to expand diabetes education services, and walk away with an actionable plan to implement a three-level care approach in their own clinical settings.

LEARNING OBJECTIVES

  • Recall the role of the Certified Diabetes Care and Education Specialist in interdisciplinary settings.

  • Evaluate strategies for expanding diabetes education services through multi-site office or clinic models to improve revenue and patient access.  

  • Identify an actionable plan to leverage interdisciplinary educator teams for comprehensive patient assessment.
