Chronic Kidney Disease and Diabetes: Community Health Worker Training Series

 

Diabetes Is Primary Case Study Modules

OVERVIEW

This interactive training series prepares community health workers to support people living with diabetes in understanding and protecting their kidney health. The series addresses chronic kidney disease (CKD) risk, screening, and next steps, with a focus on practical communication, care navigation, and community-based support strategies. Participants build skills to engage individuals in care, address common challenges, and promote person-centered, culturally responsive support. 

 

The training includes four brief, self-paced modules (approximately 15 minutes each) that may be completed in any order based on learner needs. 

 

Diabetes Is Primary® (DIP) helps primary care professionals (PCPs) on the front lines of diabetes care master knowledge and strategies to improve patient outcomes. DIP is based on the ADA’s Standards of Care in Diabetes (SOC)—the gold standard in diabetes treatment. These guidelines, updated annually, ensure that patients receive up-to-date, evidence-based care.  

The four new Diabetes Is Primary case study modules center around case studies involving different groups of people with diabetes. The case study modules feature roundtable video discussions, interactive knowledge checkpoints and are aligned with the 2025 Standards of Care updates.  

LEARNING OBJECTIVES

After completing this training series, participants will be able to: 

  • Recognize risk factors for CKD and the importance of early screening for people living with diabetes. 

  • Recognize the role of kidney health screening and ongoing monitoring, including eGFR, UACR, and CKD stages. 

  • Apply person-centered communication strategies to support engagement in kidney health, including screening, lifestyle changes, and treatment support. 

  • Identify common challenges to care and use practical strategies to support connection to clinical and community resources.

Module 1

Understanding the Basics: Diabetes and Kidney Health

Type 1 Diabetes in Adults 

This module introduces the connection between diabetes and kidney health, including key risk factors and the importance of early awareness and screening.  

Type 1 Diabetes in Adults 

 

This interactive case study module follows the story of a care team supporting an adult patient with Type 1 diabetes.  

 

Module 2 

Screening and Staging of Chronic Kidney Disease

Type 2 Diabetes in Adults

This module explains the role of kidney health screening and monitoring, including eGFR, UACR, and CKD stages, to support timely follow-up.

 

This interactive case study module follows the story of a care team supporting an adult patient with Type 2 diabetes.

Module 3 

Lifestyle and Treatment Support for Diabetes and Kidney Health

This module covers basic lifestyle and treatment concepts, including nutrition, physical activity, and medications, to support kidney health in people living with diabetes.    

This interactive case study module follows the story of a care team supporting a pediatric patient with Type 1 diabetes. 
 

Module 4 

Connecting the Dots: Community Navigation

This module focuses on identifying common challenges to care and supporting individuals in accessing clinical services and community-based resources.

FACULTY 

Erika R. Anna, MS, RDN, CD 

Erika Anna is an Outreach Specialist in the Office of Community Health at the University of Wisconsin–Madison, where she focuses on strengthening community health worker capacity locally and regionally and advancing culturally relevant food and nutrition initiatives. She also provides nutrition training for CHW cohorts through the Milwaukee AHEC CHW Training Program. A Registered Dietitian Nutritionist with more than a decade of experience in primary care and community health, her work centers communitybased participatory approaches and mixedmethods, partnering with community health workers to develop tools and practices grounded in lived experience and aligned with communityidentified needs.
