Quality Improvement for Community Health Workers

     

OVERVIEW

Quality Improvement (QI) is a systematic approach to identifying problems and finding solutions. It is a powerful tool used in health care to improve health care services and improve patient outcomes. This 3-module series is designed to introduce QI to you as a community health worker (CHWs) and provide you tools in which to enhance care in primary care offices.

 

Complete all three courses to claim your certificate of completion.

LEARNING OBJECTIVES

  • Recognize the insights that you as a CHW will bring to the QI process

  • Understand the various ways that you as a CHW can use QI to help the practices and organizations where they work 

  • Use the six steps of the QI journey to create improvements in your home and workplace 

  • Identify people in your organization with whom you can work on quality improvement projects

MODULES

MODULE 1

Introduction to Quality Improvement for Community Health Workers: Creating a Goal and Mapping the Process 

This module introduces you to the six steps in the QI process. It then focuses on the first two steps to guide you through identifying an issue, setting a QI goal, collecting data, and creating a process map to gather the details you will need to improve the problem.

MODULE 2

Continuing the QI Journey: Identifying Causes and Testable Solutions: Root Cause Identification Though Solution Implementation 

The second module focuses on the remaining steps of the QI journey by first helping you identify the root cause of a quality issue using the "5 Whys" of the QI process and then guiding you through the subsequent steps.

MODULE 3

More QI Tools for CHWs Working to Improve the Quality of Diabetes Care: Effective QI Collaboration, Presentation, and Resources 

The third module focuses on the various individuals with whom you might collaborate on QI initiatives within your organization or clinic and how to create targeted presentations for your audience. This module also contains resources from the American Diabetes Association® and the American Heart Association® to help you contribute to health care QI and enhance care for individuals with type 2 diabetes and/or heart disease.
