Community Health Worker Case Study Program 

OVERVIEW

Community health workers (CHWs) play a vital role in improving the health and well-being of the communities they serve, particularly in the prevention and management of diabetes. This short continuing education (CE) program, featuring eight case studies, is designed to enhance your knowledge of diabetes management. You'll learn to apply this knowledge to create personalized care plans for individuals, including those with co-occurring mental health issues, recognize key indicators of type 1, type 2, and gestational diabetes, and identify intervention strategies and potential complications, especially for managing type 2 diabetes in youth. By the end of this program, you'll feel empowered to guide your community toward better diabetes care and ensure access to the necessary services and support.

MODULE 1

Diabetes 101 Case Studies- Kai and Juan

MODULE 2

Gestational Diabetes Mellitus (GDM) Case Studies- Adiha and Sharon

Help Adiha navigate life with after the birth of her baby and lifestyle choices to prevent type 2. Learn how you can provide the support she needs. Join Sharon as she deals with rising glucose levels during her pregnancy, and find out how you can be a crucial part of her care.

MODULE 3

The Mental and Emotional Burden of Living with Diabetes Case Studies- Julie and Mr. Benally

Join Julie and Mr. Benally on their journeys through diabetes and its impact on their mental health. Explore effective behavioral health strategies that can enhance their well-being. See how you can influence their care plans as a compassionate CHW.

MODULE 4

Type 2 Diabetes in Youth Case Studies- Emily and James

Step into the world of two teenagers, Emily and James, as they navigate the challenges of managing type 2 diabetes. Understand the difficulties they face with a new diagnosis and learn how you, as a CHW, can provide essential support in their journey.
