Community Health Worker Diabetes Education Program   

OVERVIEW

Community health workers (CHWs) play a vital role in improving the health and well-being of the communities they serve and are critical to the prevention and management of diabetes. With four modules, this short continuing education (CE) program aims to increase CHWs’ understanding of diabetes and diabetes management, so you feel empowered to provide your community with the guidance to best manage their condition and ensure they have access to the services and care needed. 

LEARNING OBJECTIVES

  • Identify the different types of diabetes and risk factors associated with diabetes.
  • Recall gestational diabetes (GDM) screening best practices. 
  • Describe signs and symptoms of depression, anxiety, and diabetes distress. 
  • Identify both predisposed and modifiable risk factors for type 2 diabetes in youth.

MODULES

MODULE 1

Diabetes 101

Identify the different types of diabetes, risk factors, and comorbidities associated with diabetes. Select available treatment options and understand appropriate language to use when speaking about diabetes.

MODULE 2

Gestational Diabetes Mellitus (GDM)

Learn more about GDM, the risks, and recommended screening protocols. Understand glycemic target recommendations and more about common lifestyle modifications for managing diabetes during pregnancy, as well as post-delivery recommendations. 

MODULE 3

The Mental and Emotional Burden of Living with Diabetes

Learn about the connection between diabetes and mental health, plus the signs, and symptoms of depression and anxiety. Learn to summarize screening mechanisms recommended for people with diabetes and recognize when to refer individuals for advanced assessment and care.

MODULE 4

Type 2 Diabetes in Youth

Identify the risk factors for and disparities in type 2 diabetes in youth and the appropriate screening mechanisms, as well as understand the long-term impacts of type 2 diabetes and summarize best practices from diabetes prevention programs and other programs.
