Integrating CGM Into Personalized Care—CGM Series 

     

OVERVIEW

The Integrating CGM Into Personalized Care—CGM Series is designed to support the healthcare team in primary care settings by enhancing their expertise through practical, case-based training. This program will help transform continuous glucose monitoring (CGM) from a data tool into a decision-making framework that empowers clinicians, enhances engagement with individuals, and improves diabetes outcomes across diverse care settings. 

LEARNING OBJECTIVES

At the end of this activity, participants will be able to: 

  • Interpret and apply CGM metrics (including TIR, variability, and ambulatory glucose profile patterns) to inform individualized treatment plans. 

  • Utilize CGM data to guide insulin therapy decisions, including initiation, titration, and adjustment of basal and bolus therapies across diverse patient populations. 

  • Incorporate CGM insights into nutrition counseling, using data to engage patients in identifying the impact of food choices and developing sustainable behavior change strategies. 

  • Address barriers to effective CGM use and nutrition counseling, including patient access, health equity considerations, and HCP’s implementation challenges.

MODULES

Module 1—CGM Data Interpretation

This module is designed to develop skills in evaluating glycemic management using CGM-based metrics and recommended targets, as well as interpreting CGM reports to identify patterns of glycemia, variability, and hypoglycemia and hyperglycemia risk. Participants will analyze clinically meaningful glucose trends and recognize factors that may affect CGM accuracy and impact data interpretation.

Module 2—CGM-Guided Insulin Titration 

This module is designed to develop skills in using CGM data to identify clinically actionable indicators for initiating and adjusting insulin therapy. Participants will apply CGM data to optimize insulin dosing parameters, implement evidence-based insulin adjustment strategies, and differentiate person-specific factors that influence insulin titration decisions across diverse clinical populations.

Module 3—CGM-Guided Nutrition Counseling

This module is designed to develop skills in using CGM and AI-enabled insights to evaluate the glycemic impact of nutrients, dietary patterns, and popular diets while distinguishing evidence-based strategies from non-evidence-based claims. Participants will identify actionable opportunities within CGM reports, analyze the influence of food choices and eating patterns on glycemic outcomes, and select tailored nutrition counseling and CGM-based monitoring strategies to support sustainable behavior change.  

FACULTY 

William B. Horton, MD, MSc, FACE, FACP 

Dr. Horton is an associate professor of medicine in the Division of Endocrinology and Metabolism at the University of Virginia (UVA) School of Medicine. He serves as the associate program director for UVA’s Endocrinology, Diabetes, and Metabolism Fellowship Training Program and his research work focuses on testing new adjunct therapies in people with type 1 diabetes.  

Lea Dela Pena, PharmD, BCPS, CDCES 

Dr. Dela Pena is an associate professor of pharmacy practice with Midwestern University College of Pharmacy in Downers Grove, Illinois. Additionally, she is a clinical pharmacist and certified diabetes care and education specialist with Advocate Health in Aurora, Illinois.  

Nicole Patience, MS, RD, LDN, CDCES, CEDS-C 

Nicole Patience is a bilingual registered dietitian, certified diabetes care and education specialist, and certified eating disorder specialist consultant at the Joslin Diabetes Center and the Counseling Nutrition Center 360 in the Boston area. She specializes in eating disorder prevention and treatment, diabetes care, post-bariatric hypoglycemia, diabetes technology, and culturally responsive care.
