Advance Your Practice in Diabetes Prevention

     

OVERVIEW

This activity equips healthcare professionals with the knowledge and tools to identify individuals at risk for prediabetes and connect them to effective, evidence-based prevention strategies. Through a focus on screening, National DPP eligibility, and referral best practices, learners will be prepared to support improved patient outcomes through early intervention and sustained engagement in lifestyle change programs. 

LEARNING OBJECTIVES

At the end of this activityparticipants will be able to: 

  • Apply evidence-based strategies to identify and assess individuals at risk for prediabetes and determine appropriate screening approaches to support early detection 

  • Evaluate patient eligibility for the National Diabetes Prevention Program (National DPP) and facilitate connecting patients to CDC-recognized lifestyle change programs 

  • Implement effective referral practices and barrier-reduction strategies to improve patient enrollment, participation, and retention in National DPP programs

Module 1: 

What Healthcare Professionals Need to Know About Identifying and Treating People with Prediabetes 

Learn the importance of early identification of individuals at risk for prediabetes and the screening approaches used to identify patients based on age, BMI, and risk factors to enable timely intervention. 

 

Module 2: 

What Healthcare Professionals Need to Know About the National DPP and the Benefits of a Structured Lifestyle Change Program for Patients

Learn how to determine appropriate candidates for the National DPP and understand the program’s structure, benefits, and pathways for connecting eligible patients to a CDC-recognized lifestyle change program

Module 3:  

What Healthcare Professionals Need to Know About Referring Patients to Centers for Disease Control and Prevention-Recognized Lifestyle Change Program  

Learn best practices for referring patients to CDC-recognized lifestyle change programs and strategies to address barriers to enrollment, participation, and retention.
