Focus on Diabetes: Enhancing Communication Among Eye Health Specialists, Primary Care Providers, and Endocrinologists

OVERVIEW 

This module aims to strengthen collaboration among eye health specialists, primary care providers (PCPs), and endocrinologists by promoting timely, coordinated, and informed management strategies that improve outcomes for individuals at risk of diabetes-related eye disease. 

LEARNING OBJECTIVES 

By the end of this module, learners will be able to:   

  • Recognize the roles and responsibilities of PCPs, endocrinologists, and eye health specialists (optometrists, ophthalmologists, and retinal specialists) in managing diabetes-related eye disease to identify opportunities for enhanced collaboration. 

  • Recall approaches to bidirectional communication that incorporate diabetes management metrics (e.g., A1C, glycemic time in range [TIR]) into eye care plans and integrate eye exam findings into diabetes management. 

  • Identify strategies for effective referral and feedback systems to improve coordination between primary care, endocrinology, and eye health specialty clinics.
