Diabetes Complications Management
Module Description
This module is designed to address the aspects of holistic health care management, emphasizing the significance of person-centered care and evidence-based strategies in improving patient outcomes. By focusing on the identification and management of microvascular complications linked to diabetes, such as neuropathy, retinopathy, and nephropathy, participants will gain an understanding of evidence-based strategies for effectively addressing these complications, including pharmacological interventions, lifestyle modifications, and preventive measures. 
Learning Objectives

At the end of this module, participants will be able to: 

  1. Recall evidence-based strategies to managing complications associated with diabetic nephropathy. 
  2. Recall evidence-based strategies to managing complications associated with diabetic retinopathy. 
  3. Recall evidence-based strategies to managing complications associated with diabetic peripheral neuropathy and diabetic autonomic neuropathy. 
  4. Recognize the role of diabetes management in microvascular complication prevention and management. 
Module Faculty

Hope Feldman, CRNP, FNP-BC 

Hope Feldman is a board-certified Family Nurse Practitioner that practices in a Federally Qualified Health Center in Philadelphia, PA.  She works across the lifespan from newborn to older adults and enjoys giving care within the family dynamic to understand the interplay between lifestyle and health.  In this setting, primary care encompasses preventative care, urgent care, chronic illness management (including diabetes, HIV, hypertension, asthma, etc.), substance use recovery support, reproductive health, and basic psychiatric support.  Figuring out patient centric care with folks despite the challenges in the systems around us has been grounding and trust building and remains core to her approach to care.  She has served as a member of the Professional Practice committee with the American Diabetes Association (ADA) and has also served as a member, vice-chair and chair of the Primary Care Advisory Committee.  Collaboration with the American Heart Association with the Know Diabetes by Heart program.  She enjoys connecting with neighbors in various ways.
