Community Health Worker Case Study Program
This program will help you develop and support personalized care plans for the people you see living with diabetes. This program uses case studies based on real-world situations to help you apply the knowledge you gain to scenarios you may encounter in your community.

Overview:

Community health workers (CHWs) play a vital role in improving the health and wellbeing of the communities they serve and are critical to the prevention and management of diabetes.
 

The ADA’s CHW Diabetes Case Study Program is a short online course that will help you develop and support personalized care plans for the people you see living with diabetes. This program uses case studies based on real-world situations to help you apply the knowledge you gain to scenarios you may encounter in your community.  

Each 15-minute module contains two case studies focusing on community-specific health issues. This course will give you the confidence to ensure the communities you support have access to the services and care they need. 

Learning Objectives:

-Apply knowledge of diabetes management to develop personalized care plans for patients in various case scenarios, including those with co-occurring mental health issues.
-Recognize potential complications of developing type 2 diabetes in youth.
-Identify intervention strategies for managing type 2 diabetes in youth.
-Recall key indicators for type 1, type 2, and gestational diabetes 

 

Meet the Authors

Rabbi Margarita Hart
, CCHW, Board-Certified Chaplain, PCC


Margarita Hart is the Executive Director of the Indiana Community Health Workers Association (INCHWA) and Esperanza Ministries, with over 30 years of experience as a community health worker both overseas and in the U.S. She advocates for the CHW workforce and immigrant populations, shaping public health strategies at local, state, and national levels. As a multilingual medical interpreter and Board-Certified Christian Chaplain specializing in end-of-life care, Margarita is dedicated to culturally sensitive service in central Indiana. Additionally, she is a PCC-accredited Leadership and EQ Coach, motivational speaker, and author, helping leaders and communities thrive.

 

Amit Gupta, DNB, FACE, FRCP, FACP 

Dr. Amit Gupta is a senior doctor with 12 years of experience, serving as the Medical Director of the Centre for Diabetes Care in Greater Noida. He holds prestigious fellowships, including FRCP (Glasgow and Edinburgh), FACP, and FACE, and has completed advanced studies in diabetes management from CMC Vellore and Cardiff University. Dr. Gupta has served on the Membership Advisory Board of the American Diabetes Association (2021-2023) and the National Executive Council of the Research Society for the Study of Diabetes in India. Recognized nationally and internationally, he has received multiple awards, including the Rising Star Award from the Indian Academy of Diabetes and the Times of India Health Icon Award. Dr. Gupta has contributed to diabetes research through numerous publications and lectures and was a key contributor to India's first Joint Guidelines for diabetes management.


Carey Frazier, CCHW

Carey Frazier, the Assistant Director of the Indiana Community Health Worker Association (INCHWA) and owner of TLC Transformational Life Coaching, has been a community health worker (CHW) for over ten years. Her passion for giving people a voice and empowering others to achieve goals is evident in the results of those she works with. Carey is actively involved in creating a safe space for CHWs where they can learn more skills, advance their career, advocate for policy, and care for their mental health. As an ACC-accredited Nutrition and Life Coach, she provides consulting on stress management, nutritional healthy eating, creating more motion in our day, how to make change when dealing with fear, and other life habits to CHWs and CHW Management. Carey has a diverse background of training in the areas of Mental Health, Maternal and Child, Substance Use Disorder, Chronic Health, Navigation of Insurance, both private and Medicaid, Fetal Alcohol Syndrome, Crisis Response, Adverse Childhood Experiences, Suicide Prevention, and Domestic Violence.
