Video Summary
This webinar focused on continuous glucose monitoring (CGM) in pregnancy, hosted by ADA moderators Dr. Gwendolyn Jack and Dr. Cassandra Henderson, with presentations by Dr. Pamela Dolnik and Dr. Celeste Dernwald.

The speakers reviewed why tight glycemic control matters in pregnancy, including the increased risks of congenital anomalies, preeclampsia, cesarean delivery, large-for-gestational-age infants, neonatal hypoglycemia, and NICU admission. They explained how older measures like A1c and finger-stick monitoring have limitations in pregnancy, and how CGM can provide richer data on fasting, nocturnal, postprandial, and between-meal glucose patterns.

Evidence was strongest for type 1 diabetes in pregnancy, especially from the CONCEPT trial, which showed improved A1c, more time in range, and better neonatal outcomes with CGM use. For gestational diabetes, newer trials showed mixed results: some found no clear neonatal benefit, while others showed improved time in range and reductions in large-for-gestational-age outcomes. For type 2 diabetes, the presenters noted that data are still limited, but ongoing trials may clarify its role.

Dr. Dernwald provided practical guidance on interpreting CGM reports, including reviewing sensor wear, time in range, time below range, variability, and daily patterns. She emphasized setting pregnancy-specific goals, avoiding alarm fatigue, and using CGM data to guide individualized treatment adjustments.

The session concluded with a Q&A on hypoglycemia thresholds, postpartum screening, and possible future use of combined glucose-ketone monitoring.
