To avoid manual chart abstraction, the program built a population registry using electronic feeds routed through the state health information exchange (MIHIN) and supplemented with claims (BCBSM and Medicaid), while acknowledging major data completeness and governance challenges.<br /><br />Early QI efforts focused on three pillars—guideline-directed medications, low-carbohydrate/lifestyle strategies, and CGM—then evolved into performance measures aligned with HEDIS (A1C control, urine albumin-creatinine screening) and an endocrinology CGM interpretation/billing metric. The presenters emphasized human-centered design, education events, widely shared free resources, and value-based incentives as key “secret sauce” elements, along with ongoing challenges such as medication costs, team variability, EMR differences, and slow system change.
