Video Summary
The panel discussed the need to reclaim the full spectrum of obesity care rather than treating GLP-1 medications as the sole standard of care. Speakers emphasized that obesity is a complex, chronic, and changing disease, so treatment should be individualized around health goals, patient preferences, tolerability, and access.

Dr. Catherine Saunders highlighted the continuing role of first-generation medications, including phentermine, orlistat, phentermine/topiramate, and naltrexone/bupropion. Though their average weight-loss results are generally lower than those of newer drugs, some patients respond substantially. These medications can also complement newer treatments, address specific symptoms such as cravings, or provide alternatives when patients experience side effects, lack coverage, or do not respond.

Dr. Robin Pashby described the psychological and cultural effects of powerful obesity medications. Patients may experience relief from hunger and food preoccupation, alongside anxiety about weight regain, changes in identity and relationships, stigma, and unequal access. Behavioral health care should support emotional adjustment and monitor disordered eating, rather than focus only on adherence or habit change.

Dr. Rich Peterson presented metabolic and bariatric surgery as an effective, generally safe option that remains underused. He argued that surgery and medication can work together, with integrated care guiding patients through treatment rather than positioning surgery as a last resort.

The discussion stressed preparing patients for the possibility of nonresponse, explaining alternatives in advance, and using multidisciplinary care. Emerging medications, lower drug costs, and endoscopic procedures may expand options, but no single approach replaces the need for a broad, personalized treatment toolkit.
